On your first visit with us, please make sure you arrive 20 minutes before your scheduled appointment time so that we can verify your insurance and have you complete necessary paperwork. It will save time if you download the new patient forms and complete them in full prior to the appointment. It is important to bring your child’s current insurance card. If you have not arranged for your prior physician to send your child’s medical records, please bring a copy with you.
